According to Local Government Code, Chapter 176, a vendor, person or an agent of a person who contracts or seeks to contract for the sale or purchase of property, goods, or services with Hawley ISD, must file a completed Conflict of Interest Questionnaire (COI) with the HISD Business Office. Any person or vendor not currently registered as a vendor must fill out the HISD Vendor Application below, which includes the Vendor Request Form, W-9, and the Conflict of Interest Questionnaire. If circumstances change that would require a change to the COI, it is the responsibility of the Vendor to file a new form with the Business Office. Submit completed application to Mitzi Culpepper at the HISD Business Office: mculpepper@hawley.esc14.net.
